Service Coordinator
Every participant in the HCS program is matched with a service coordinator, whose role is to:
Develop and oversee the execution of the Individual Service Plan (ISP), ensuring that personalized care is coordinated both within the HCS framework and through additional services.
Individual Socialization Skills Program
This program is structured to aid participants with daily social interactions and includes involvement in leisure activities. It aims to enhance their abilities in independence, self-sufficiency, social engagement, everyday life skills, and setting of personal achievement targets.
Residential Services
We offer a homely, secure setting for ongoing support in daily living. Customized to empower individuals to live as independently as possible while receiving the care they require. Our spaces are crafted to promote each person's growth and wellness.
Group Home Living
We provide a choice of residing in our State-certified HCS Residential Group Homes. With 3 to 4 bed configurations, they are structured within the scope of the Home and Community-Based Services Medicaid waiver program.
Host Home/Companion Care
Our service excels in fostering companion care within the inviting environment of a family-style home. Balancing autonomy with support, this option is committed to safety and enriching the daily routines, while emphasizing valuable, individualized relationships.
Respite Services
Supplying temporary assistance to primary caregivers, our respite options provide reliable, caring substitution to ensure the continuity of care. This gives families the opportunity to refresh, with full confidence that their cherished ones receive the utmost care.
